Screening Numbers obtained before January 1, 2012 have expired. You must complete screening if you have not been screened since this date.
To inform PYH of a screening confirmation number you previously obtained, click on this link. Please DO NOT send an email. We use the form to track the numbers.
If you need to obtain a new screening number, please follow ALL the instructions below:
1) Go to the NYSAHA Screening Policy website for information on the screening policy.
2) To start the screening process, click on this link: http://www.commercialinvestigationsllc.com/VSSNYSAHA
3) When you have completed the screening, you will receive an email with the Subject:
Background Report Order #NNNNNNNN Successful
4) Submit your Background Report Order to PYH
- Click on the NYSAHA Screening Confirmation Number
link (found on the Online Forms
tab on the left side of the PYH homepage).
5) If you want PYH to reimburse you, please send your name, confirmation number and a self-addressed stamped envelope to:
